Qualified Social Worker - Homefirst & Urgent Response Team - Swindon

? Location: Swindon (Office-based 2 days/week, urgent client visits as required)
? Contract: Ongoing
? Hours: Full-time
? Rate: £37 per hour

Eden Brown Synergy is recruiting for a Qualified Social Worker to join the Homefirst & Urgent Response Team in Swindon. This is a fast-paced and dynamic team supporting hospital discharge and admission avoidance.

Key Responsibilities:
? Support hospital discharges and prevent unnecessary hospital admissions
? Respond to urgent referrals and complete assessments at short notice
? Work collaboratively within a multi-disciplinary team
? Conduct Care Act assessments and develop person-centred care plans
? Ensure timely and effective interventions for service users

Requirements:
?? Qualified Social Worker with relevant experience
?? Strong background in hospital discharge and admission avoidance
?? Experience in multi-disciplinary working (evidence preferred)
?? Must be a car driver and live within 45 minutes of Swindon
