Qualified Social Worker - Hospital Team - Telford
? Location: Princess Royal Hospital (Hospital Team Base, Client Visits & Home)
? Contract: 3 months (approx.)
? Hours: 37 per week
? Rate: £34 per hour

Eden Brown Synergy is recruiting for a Qualified Social Worker to join the Hospital Team in Telford. This role is based at Princess Royal Hospital and involves working within a fast-paced environment to support hospital discharges and enablement services.

Key Responsibilities:
? Conduct Enablement Reviews and Care Act Assessments
? Complete Mental Capacity Act Assessments and Best Interest Decisions
? Work within an asset-based approach and complete CHC checklists
? Apply a person-centred approach to assessments and support planning
? Manage a fast-paced caseload efficiently

Requirements:
?? Qualified Social Worker with relevant experience
?? Previous experience in enablement services is essential
?? Familiarity with LAS case management system is preferable
?? Must be able to drive and have access to a car
