? Hiring: Qualified Social Worker - Continuing Health Care Team ?

? Location: 1 Saxon Gate East, Milton Keynes MK9 3EJ
? Pay Rate: £32 - £35 per hour (Umbrella)

Milton Keynes Council is looking for a Qualified Social Worker to join the Continuing Health Care Team. This is an exciting opportunity to work alongside NHS colleagues, providing high-quality care and assessments for individuals with complex health and social care needs.

Role Overview:
As a Social Worker in the Continuing Health Care Team, you will be responsible for: ? Undertaking assessments under the Care Act 2014 and NHS Continuing Health Care Guidance.
? Collaborating with NHS colleagues to holistically assess and challenge eligibility/funding decisions.
? Completing safeguarding enquiries, mental capacity assessments, and best interest decisions.
? Managing a complex caseload and ensuring assessments are completed within the 28-day time period.
? Providing support to colleagues in adult social care with Continuing Health Care cases.
? Attending regular supervision meetings to ensure informed decision-making.
? Promoting professional leadership and development within the team.

About You:
? Qualified Social Worker with experience in Continuing Health Care or Adult Social Care.
? Ability to manage a complex caseload and meet time-sensitive deadlines.
? Strong understanding of Care Act 2014, NHS Continuing Health Care Guidance, and safeguarding.
? Experience with mental capacity assessments and best interest decisions.
? Excellent communication skills, able to collaborate effectively with multi-disciplinary teams.
