? Hiring: Qualified Social Worker - Ageing Well Team ?

? Location: Southend-on-Sea
? Salary: £37,938 - £43,693 per annum

Are you an experienced Social Worker with a passion for supporting older adults to live independently? We are currently looking for a qualified social worker to join the Ageing Well Team in Southend-on-Sea.

Role Overview:
As a Social Worker within the Ageing Well Team, you will:
? Work with older adults and those with complex care needs to promote independence and well-being.
? Conduct assessments, reviews, and care planning to ensure the best possible support.
? Collaborate with families, healthcare providers, and other agencies to deliver person-centred care.
? Help to ensure care packages are delivered in line with the council's policies and Care Act 2014.
? Provide advice and support to families and carers, offering guidance on available resources and options.

About You:
? Qualified Social Worker with experience working with older adults or complex cases.
? Knowledge of adult safeguarding and care management.
? Ability to work independently and within a team.
? Strong communication skills and the ability to build relationships with service users and partners.
? Enthusiastic and dedicated to supporting older adults to live independently.
