Adult Social Worker - 2- 4 years (Grade H)

We are recruiting a qualified, skilled and committed Social Worker with experience in adult social care to work in Social Care Direct (which is our Front Door Team).?

Social Care Direct is the first point of contact for all initial enquiries about social care. You will put the person at the heart of the assessment and involve their families, friends and communities around them to help improve their lives and help them meet their goals.?

* You will assess and review their care and support needs.?

* You will provide information, advice and care to adults with care and support needs so that they have the maximum opportunity to regain their independence or become less dependent on other people or services.?

* You will provide a high standard of social work arising out of the Council’s legal duties and powers in accordance with divisional and area policy, procedures, guidelines and the Code of Practice for Social Care Workers.?

* You will undertake the more complex assessments of care needs, risks and mental capacity for people who require professional input.?

* You will determine Care Act eligibility thresholds?

About you?

The Social Care Direct Team require:?

* Experienced social workers with at least 4 years’ experience?

* With experience in working in a strength based way with individuals?

* Who is able to identify the presenting problem early on, therefore offering focused and effective intervention ?

* Has worked or has a preference in working in teams that offers short term intervention and work at a fast pace?

* Is IT literate and ideally has had experience using Mosaic ?



We offer 36 hours a week, hybrid working with a mixture of office and home working, although staff are welcome to be office based fulltime if they wish.
