Greenwich - Qualified Social Worker - Hospital Team

? Rate: £32 per hour

Job Overview:
The Royal Borough of Greenwich is seeking an experienced Qualified Social Worker to join the Hospital Team. The role focuses on facilitating safe and timely discharges from hospital, conducting Care Act assessments, and working within a multi-disciplinary team to support vulnerable adults.

Key Responsibilities:
Conduct Care Act 2014 assessments for hospital patients.
Facilitate safe hospital discharges, ensuring the right care packages are in place.
Work within safeguarding procedures to assess and mitigate risks.
Complete Mental Capacity Assessments (MCA) and Best Interest Decisions when required.
Work closely with NHS professionals, families, and community services to support patient transitions.
Maintain accurate records using the MOSAIC system or equivalent.
Requirements:
? Qualified Social Worker with Social Work England registration.
? Previous hospital discharge planning experience.
? Strong knowledge of the Care Act 2014, MCA 2005, and Safeguarding procedures.
? Ability to work in a fast-paced hospital setting and make timely decisions.
? Experience using MOSAIC or similar case management systems is preferred.

Additional Information:
This is an on-site role with no remote working.
The position is full-time (Monday-Friday, 9 AM - 5 PM).
