Job Description
We are currently recruiting for an experienced Occupational Therapist to join the Early Intervention Integrated Services in Wigan. This role involves working across Reablement, Equipment and Adaptations, and Assistive Technology, supporting both children and adults to achieve maximum independence in their home and community environments.
You will assess and manage complex physical, learning, and mental health needs, working within a multi-disciplinary team to develop person-centered solutions that promote independence and well-being.
Key Responsibilities:
Conduct specialist assessments, treatment, and management of adults and children with diverse and complex needs.
Support service users experiencing reduced independence with reablement programs, assistive technology, and home adaptations.
Work collaboratively with social workers, health professionals, carers, and families to deliver an integrated service.
Identify and implement equipment and adaptation solutions, ensuring the best possible outcomes for service users.
Provide specialist input for complex cases, including risk assessments, care planning, and intervention strategies.
Maintain accurate case records in line with legal and service requirements.
Contribute to training, supervision, and mentoring of unqualified staff.
Participate in case conferences, safeguarding meetings, and multi-agency planning.
Requirements:
Degree or Diploma in Occupational Therapy.
HCPC Registration.
Strong knowledge of the Care Act 2014, Children and Families Act 2014, and Disabled Facilities Grant legislation.
Experience in assessing and implementing reablement programs, equipment, and home adaptations.
Ability to work in partnership with individuals, families, and external agencies.
Understanding of safeguarding procedures for adults and children.
Excellent communication and IT skills.
Moving and Handling training (or willingness to complete).
