Stockport - QSW - Mental Health Hospital discharge

Job Description

We are currently recruiting for an experienced Mental Health Hospital Discharge Social Worker to join our newly established All-Age Adult Mental Health Hospital Discharge Team in Stockport. This is an exciting opportunity to work collaboratively with Pennine Care colleagues to improve mental health services and outcomes for individuals leaving hospital.

Key Responsibilities:
Support older and working-age adults with functional or organic mental health diagnoses to ensure safe and timely discharges from hospital.
Work closely with individuals, carers, health professionals, advocates, and third-sector partners.
Complete Care Act assessments, support plans, and risk assessments.
Undertake Mental Capacity Assessments, chair Best Interests Meetings, and write Social Circumstances Reports.
Attend Mental Health Tribunals and contribute to multidisciplinary decision-making.
Identify and commission appropriate community care and support services.

Requirements:
Qualified Social Worker registered with Social Work England.
Strong knowledge of the Mental Health Act and Care Act.
Experience in mental health social work and hospital discharge planning.
Ability to take a strengths-based approach to care planning.
Must be a car driver with access to a vehicle for work purposes.
