Job Description
We are looking for an experienced Senior Social Worker to join our Adult Social Care team in St Helens. This role involves undertaking needs-led assessments, providing specialist support, and ensuring high-quality care for adults and their carers.
Key Responsibilities:
Complete complex multi-disciplinary assessments and provide ongoing case management for individuals with unstable and unpredictable conditions.
Develop personalised support plans based on identified needs.
Participate in brokering and tendering processes as required.
Conduct carers' assessments, addressing their needs separately from the individuals they support.
Work to maintain individuals in the community, managing challenging behaviours and associated risks, with guidance from senior management.
Requirements:
Qualified Social Worker with relevant experience in Adult Social Care.
Strong assessment, risk management, and case management skills.
Ability to work collaboratively with multi-agency teams.
Commitment to improving outcomes for adults and their carers.
