About the Role:
Sutton Council is seeking an experienced Qualified Social Worker to join the Adult Community Team. You will work with vulnerable adults and their carers, providing assessments, planning support, and ensuring appropriate interventions under the Care Act 2014 and other relevant legislation.

Key Responsibilities:
?? Complete Social Care Assessments, Support Planning & Reviews
?? Conduct Safeguarding Enquiries, Risk Assessments & Protection Planning Meetings
?? Undertake Mental Capacity Assessments & Best Interest Decisions
?? Prepare Court of Protection applications where necessary
?? Carry out Carers Assessments & Reviews
?? Participate in NHS Continuing Healthcare (CHC) Checklist & DST Meetings
?? Work closely with multi-disciplinary teams, health professionals & voluntary services

What We're Looking For:
? Qualified Social Worker (SWE registered)
? Strong experience in adult social care & community settings
? Knowledge of Care Act 2014, Mental Capacity Act, and Safeguarding procedures
? Experience in Court of Protection applications and CHC processes
? Ability to work collaboratively with professionals and families
? Strong report writing and decision-making skills
