? Location: Stockwell Close, Bromley BR1 3UH
? Rate: £32 per hour
? Work Arrangement: Fully on-site

Join Bromley's Hospital Social Work Team!
An exciting opportunity has arisen for an experienced Qualified Social Worker to join the Hospital Team at Princess Royal University Hospital in Farnborough, Bromley. This role involves working on-site with health professionals to facilitate safe and timely hospital discharges for vulnerable adults.

Key Responsibilities:
?? Hospital Discharge & Risk Management

Work closely with Health colleagues to facilitate safe hospital discharges.
Attend daily board meetings to discuss patient flow and discharge planning.
Reduce the risk of social admissions to hospital or temporary placements.
?? Assessments & Safeguarding

Complete Care Act 2014 and Mental Capacity Act 2005 assessments.
Arrange care support packages to enable a safe return home.
Work with Locality Teams for follow-up care after discharge.
?? Working with Adults & Partner Agencies

Support Younger Adults with physical disabilities & sensory impairments.
Support Older People to live independently while managing risks.
Work within a multidisciplinary team, ensuring effective communication and collaboration with partner agencies.
About You:
? Qualified Social Worker (Social Work England registered)
? Experience in hospital discharge planning & risk management
? Strong knowledge of Care Act 2014 & Mental Capacity Act 2005
? Ability to complete assessments & safeguarding enquiries (Section 42)
? Excellent communication and negotiation skills
? Flexible, committed, and able to work to high-quality standards
