Job Advert: Qualified Social Worker (QSW) - Transitions Team

Location: Wolverhampton
Rate: £30 per hour

We are seeking an experienced Transitions Social Worker to join the SEND Family Help Service in Wolverhampton. This role involves supporting young adults (17.5 - 25 years old) as they transition from children's services into adult services.

Key Responsibilities:
Work with multi-agency teams, including health and education, to develop plans that support young adults in accessing their communities.
Conduct Mental Capacity Act and Care Act assessments to ensure individuals receive the right support.
Support with Deprivation of Liberty Safeguards (DoLS)-training available if required.
Provide high-quality interventions to ensure smooth transitions into adult services.

Essential Requirements:
Qualified Social Worker registered with Social Work England.
Minimum of three years permanent experience in a relevant role (per DfE requirements).
Experience in mental capacity assessments and Care Act duties.
