? Pay Rate: £32 p/h
? Full-Time | Office-Based Role (No Remote Working)

We are seeking an experienced Qualified Social Worker (QSW) to work within the Hospital Team based in Basildon Hospital. This role requires strong assessment skills, knowledge of hospital discharge processes, and experience in safeguarding.

Key Responsibilities:
?? Hospital Discharge & Mental Capacity Assessments:

Undertake timely hospital discharge planning assessments, ensuring safe and effective transitions back into the community.
Conduct Mental Capacity Assessments (MCA) and ensure compliance with the Care Act 2014 & Mental Capacity Act 2005.
?? Safeguarding & Multi-Agency Working:

Identify and respond to safeguarding concerns, following Section 42 Enquiries.
Work in close partnership with NHS teams, occupational therapists, and other health and social care professionals to develop support plans.
?? Case Management & Systems:

Maintain accurate case records using Liquidlogic Adults System (LAS).
Provide professional social work intervention, support planning, and risk management for vulnerable adults.
About You:
? Registered Social Worker with Social Work England.
? Previous experience working in a hospital discharge team or adult social care setting.
? Strong knowledge of Care Act 2014, Mental Capacity Act, and Safeguarding Adults Framework.
? Ability to work in a fast-paced environment and manage complex cases.
? Experience using Liquidlogic (LAS) is highly desirable.
? Must be able to work full-time in the office (no remote working available).
