Job Description

We are currently seeking an experienced Senior Practitioner Social Worker to join the Older Adult Community Mental Health Team (CMHT) in Trafford, part of Greater Manchester Mental Health NHS Foundation Trust.

Role Details:
Location: Older Adult CMHT, Crossgate House, Sale
Working Arrangements: Office-based
Hours: Full-time
Contract Length: 3 months
Rate: £32 per hour
Driving Required: Yes (clean driving license and own transport needed)
DBS Requirement: Enhanced

Role Overview:
As a Senior Practitioner Social Worker, you will provide social work leadership within a multi-disciplinary team consisting of Consultant Psychiatrists, Clinical Psychologists, Community Mental Health Nurses, Occupational Therapists, and STR Workers. Your role will focus on supporting older adults with serious and enduring mental illness, including dementia, promoting social inclusion and recovery to enhance independent living.

Key Responsibilities:
Lead and promote mental health social work within the team
Work closely with service users, families, and professionals to provide high-quality care
Support individuals with serious mental illness and dementia to live independently
Contribute to the multi-disciplinary approach within the CMHT

Who We're Looking For:
Qualified and experienced Social Worker with a strong background in older adult mental health
Skilled in social work leadership and multi-disciplinary collaboration
Committed to recovery-focused, person-centered care
Holds a clean driving license and is able to travel across the borough as needed
