Job Description

We have an exciting opportunity to join our Prevention & Personalisation Team in Derbyshire, offering a competitive hourly rate of £34.

About the Role:
As a Qualified Social Worker within our Prevention & Personalisation Team, you will play a key role in promoting independent living and personalized care for adults. You will work closely with individuals to assess their needs, develop personalized support plans, and coordinate services to enhance their quality of life.

Key Responsibilities:

Conducting assessments to identify individual needs and risks.
Developing personalized support plans in collaboration with service users, families, and carers.
Providing advocacy and support to empower individuals in decision-making.
Coordinating with community resources and services to facilitate personalized care.
Monitoring and reviewing support plans to ensure effectiveness and adaptability.

Requirements:

Qualified Social Worker with a recognized social work qualification (e.g., BA Social Work, MA Social Work, DipSW).
Registered with Social Work England.
Experience working in adult social care settings, preferably within prevention or personalization teams.
Strong assessment, communication, and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Commitment to promoting independence, choice, and dignity for service users.
