Job Description

We have an exciting position available in Bolsover, Derbyshire, offering competitive pay at £34 per hour.

About the Role:
As a Qualified Social Worker, you will play a vital role in supporting vulnerable adults within the community of Bolsover. Your responsibilities will include assessing the needs of individuals, developing care plans, and providing ongoing support and guidance. You will work closely with multidisciplinary teams to ensure the well-being and safety of service users.

Key Responsibilities:

Conducting assessments to identify care needs and risks.
Developing and implementing care plans in collaboration with service users and their families.
Monitoring and reviewing care plans to ensure effectiveness.
Providing advocacy and support to empower individuals in making decisions about their care.
Working collaboratively with external agencies and healthcare professionals.

Requirements:

Qualified Social Worker with a relevant social work qualification (e.g., BA Social Work, MA Social Work, DipSW).
Registered with Social Work England.
Previous experience in adult social care settings is desirable.
Strong assessment and communication skills.
Ability to work autonomously and as part of a team.
Commitment to providing high-quality, person-centered care.
