Job Title: Social Worker - Disabilities Team
Location: Welshpool, Powys
Hours: 18.5 hours per week
Salary: £35 per hour

We are currently seeking a dedicated and experienced Social Worker to join our Disabilities Team based in Welshpool. This is an excellent opportunity to make a meaningful impact in the lives of individuals with disabilities and their families.

Responsibilities:

Assessing the needs of individuals with disabilities and their families to determine appropriate support and interventions.
Developing and implementing care plans in collaboration with service users, their families, and other professionals.
Providing advocacy and support to ensure individuals have access to necessary resources and services.
Monitoring and reviewing the effectiveness of care plans, making adjustments as needed.
Working collaboratively with colleagues and partner agencies to ensure integrated and holistic support for service users.
Requirements:

Qualified Social Worker with registration with Social Care Wales.
Experience working with individuals with disabilities and their families in a social care setting.
Strong assessment, communication, and interpersonal skills.
Ability to work effectively both independently and as part of a team.
Commitment to promoting the rights and autonomy of individuals with disabilities.
