Hourly Rate: £29.80

Role Overview:
We are seeking a dedicated and compassionate Social Worker to join our Adult Social Care team in Oxfordshire. As a Social Worker, you will play a pivotal role in assessing the needs of adults requiring support and developing care plans tailored to their individual circumstances. You will work closely with service users, their families, and a multi-disciplinary team to promote independence, well-being, and safeguarding.

Key Responsibilities:

Conducting comprehensive assessments of adults care and support needs.
Developing and implementing care plans in collaboration with service users and their support networks.
Providing advice, support, and advocacy to service users to help them navigate the social care system.
Liaising with external agencies and professionals to coordinate holistic care provision.
Monitoring and reviewing care plans to ensure they remain effective and responsive to changing needs.
Safeguarding vulnerable adults and taking appropriate action in cases of suspected abuse or neglect.
Requirements:

Degree in Social Work or equivalent qualification.
Registration with Social Work England.
Proven experience working as a Social Worker within adult social care settings.
Strong assessment and care planning skills.
Excellent communication and interpersonal abilities.
Ability to work effectively as part of a multi-disciplinary team.
Commitment to promoting equality, diversity, and inclusion in service delivery.
Knowledge of relevant legislation and policies governing adult social care.
