Job Description

We are currently seeking a passionate and experienced Qualified Social Worker (QSW) to join our Adult Care & Community team in Lincolnshire. In this role, you will have the opportunity to work with adults, conducting strength-based assessments and providing person-centered support to help individuals maintain their independence.

Responsibilities:

Conducting comprehensive strength-based assessments to identify individual needs and preferences.
Implementing person-centered care plans that promote independence and well-being.
Demonstrating a strong understanding of the Community Care Act, Mental Capacity Act, and other relevant social care legislation.
Ensuring the safeguarding of vulnerable adults through proactive risk assessment and intervention
.
Requirements:

Qualified Social Worker with relevant registration and experience in adult social care.
Sound knowledge and understanding of the Community Care Act, Mental Capacity Act, and other social care legislation.
Ability to work in a person-centered way, respecting the autonomy and choices of individuals.
Strong communication and interpersonal skills, with the ability to build rapport with service users and multidisciplinary teams.
Commitment to promoting independence and safeguarding the well-being of vulnerable adults.
