Lincolnshire - QSW - Acute Hospital £35ph

Job Description

We are seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team at a busy Acute Hospital in Lincolnshire. As a QSW, you will play a vital role in supporting and facilitating discharges, working collaboratively within a multidisciplinary team that includes professionals from the health sector and third-party organizations.

Responsibilities:

Conducting comprehensive assessments and developing tailored care plans while adhering to legal obligations under the Care Act.
Providing support and guidance to patients, their families, and caregivers throughout the discharge process.
Participation in Multidisciplinary Team (MDT) meetings to discuss complex cases, including those related to safeguarding and the Mental Capacity Act.
Working on-site at the hospital and being available to cover weekend shifts on a rotational basis, with corresponding days off during the following week.

Requirements:

Qualified Social Worker with relevant registration and experience in a hospital setting.
Sound knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding procedures.
Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
Flexibility to work weekend shifts as part of a rotational roster.
Strong organizational skills and the ability to manage complex caseloads effectively.
