Position Overview:
We are seeking a dedicated and compassionate Social Worker to join our Young Adults Transitions team. This role involves working with young adults aged 16-25 who are transitioning from childrens services to adult support services. You will play a crucial role in assessing their needs, providing support, and helping them navigate the challenges of adulthood.

Key Responsibilities:

Conducting assessments to determine the needs and goals of young adults transitioning to adult services.
Developing and implementing care plans in collaboration with service users, their families, and relevant stakeholders.
Providing advocacy and support to ensure that young adults receive the services and resources they require to achieve their goals.
Working closely with other professionals, including health care professionals, educators, and support workers, to coordinate services and promote integrated care.
Monitoring and reviewing the progress of service users and adjusting support plans as necessary.
Safeguarding vulnerable individuals and taking appropriate action in cases of suspected abuse or neglect.
Maintaining accurate and up-to-date records in accordance with Bristol City Council policies and procedures.
Qualifications and Experience:

Degree in Social Work or equivalent qualification recognized by Social Work England.
Registered Social Worker with Social Work England.
Experience working with young adults in a social care setting, preferably in transitions or youth services.
Knowledge of relevant legislation, policies, and procedures related to adult social care and transitions.
Strong assessment, communication, and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
