Hourly Rate: £31.27 per hour

Job Summary:
Join the innovative Adult Neurodiversity Team at Worcestershire County Council as a Social Worker. We are excited to expand our team and are seeking two experienced individuals to work with adults who have autism, ADHD, learning difficulties, and other associated conditions. Working within a strength-based model, you will collaborate with a multidisciplinary team to create personalized care plans that promote independence, choice, and well-being for individuals. This role involves assessments, reviews, capacity assessments, and safeguarding, providing an opportunity to make a meaningful impact in the lives of those you support.

Main Duties & Responsibilities:

Conduct comprehensive assessments of individuals needs, strengths, and preferences, considering their neurodiversity and associated conditions.
Develop and implement personalized care plans in collaboration with individuals, their families, and the multidisciplinary team, utilizing a strength-based approach.
Facilitate reviews of care plans to ensure they remain responsive to individuals changing needs and aspirations.
Undertake capacity assessments and safeguarding interventions as necessary, advocating for the rights and well-being of individuals.
Work collaboratively with partner agencies and community resources to coordinate support services and promote individuals social inclusion and community participation.
Maintain accurate and up-to-date records of interventions, assessments, and outcomes in accordance with statutory requirements and organizational policies.
Requirements:

Qualified Social Worker with current registration with Social Work England.
Significant experience working with individuals who are neurodivergent and their families in a social work capacity.
Demonstrated experience in conducting assessments, reviews, capacity assessments, and safeguarding interventions.
Excellent communication and interpersonal skills, with the ability to build rapport and establish effective relationships with individuals, families, and colleagues.
Ability to work autonomously and as part of a team, demonstrating resilience and flexibility in a dynamic work environment.
Commitment to promoting the rights, independence, and well-being of individuals with neurodiverse conditions.
