Job Summary:
Join the dedicated team at Barnet and Chase Hospital as a Social Worker. We are seeking individuals who are passionate, resilient, and experienced in social work within a hospital environment. As a Social Worker, you will play a crucial role in supporting individuals, their families, and carers through assessment, care planning, and coordination of support services. This position requires full-time on-site presence at Barnet Hospital, Hertfordshire.

Main Duties & Responsibilities:

Conduct comprehensive assessments of individuals needs, considering their personal circumstances, and develop appropriate care plans in collaboration with relevant stakeholders.
Provide ongoing support and intervention to individuals and their families, addressing safeguarding concerns and promoting well-being.
Liaise effectively with multi-disciplinary teams within the hospital setting to ensure holistic care provision and facilitate discharge planning.
Maintain accurate and up-to-date records in accordance with statutory requirements and organizational policies.
Stay abreast of relevant legislation, policies, and best practices in social work, applying this knowledge to inform practice and decision-making.
Engage in reflective practice and continuous professional development to enhance skills and effectiveness in the role.
Requirements:

HCPC registration as a Social Worker.
Completion of ASYE (Assessed and Supported Year in Employment).
Previous experience working as a Social Worker in a hospital environment.
Thorough understanding of relevant legislation and policies governing social work practice.
Strong interpersonal and communication skills, with the ability to build effective relationships with individuals, families, and colleagues.
Ability to work autonomously and as part of a team, demonstrating resilience and adaptability in a dynamic work environment.
Additional Information:
This is a full-time position based at Barnet Hospital. Candidates should reside within a reasonable commuting distance to ensure on-site presence five days a week. Remote work options are not available for this role.
