An exciting opportunity has arisen for a Qualified Social Worker (QSW) specializing in Mental Health to join our dynamic team in Doncaster. This role specifically focuses on supporting older people with mental health issues, particularly those transitioning from a Mental Health ward setting.

Role Overview:
As a Qualified Social Worker specializing in Mental Health for older people, you will play a crucial role in supporting individuals as they transition from Mental Health ward settings back into the community. Your responsibilities will include conducting assessments, developing care plans, providing counseling and support, coordinating with healthcare professionals, and ensuring the smooth transition and integration of service users into community settings.

Key Responsibilities:

Conduct comprehensive assessments of service users needs, taking into account their mental health condition and social circumstances.
Develop individualized care plans in collaboration with service users, their families, and multidisciplinary teams.
Provide ongoing support and counseling to older individuals with mental health challenges, promoting independence and empowerment.
Liaise with healthcare professionals, including psychiatrists, psychologists, and community mental health teams, to ensure coordinated care and support.
Facilitate access to relevant community resources and services, such as housing support, day centers, and social activities.
Monitor and review the progress of service users, adjusting care plans as necessary to meet their evolving needs.
Maintain accurate and up-to-date records in accordance with professional standards and organizational policies.

Requirements:

Qualified Social Worker (QSW) with relevant experience in Mental Health, preferably with older people.
Registration with the Health and Care Professions Council (HCPC) or eligibility to register.
Strong assessment and interpersonal skills, with the ability to engage effectively with older individuals experiencing mental health challenges.
Knowledge of relevant legislation, policies, and procedures relating to Mental Health and Adult Social Care.
Ability to work collaboratively within multidisciplinary teams and communicate effectively with stakeholders.
Commitment to promoting the rights, dignity, and well-being of older people with mental health needs.
