Job Description

Are you a dedicated and experienced Qualified Social Worker seeking a dynamic role in Adult Services? Join our busy Short Term Assessment and Reablement Team in Derbyshire and play a vital role in supporting adults to regain independence and achieve their goals. This position offers hybrid working arrangements, providing a balance between office-based and community visits.

Pay rate upto £34 p/h

Key Responsibilities:

Conduct assessments of adults with short-term care and support needs, focusing on promoting independence and facilitating reablement.
Develop and implement person-centered support plans in collaboration with service users, their families, and other professionals, ensuring a holistic approach to care.
Provide practical support and guidance to individuals and their families, empowering them to identify and achieve their goals for independence and well-being.
Undertake community visits to assess individuals living environments and identify any additional support needs or risks.
Work closely with multidisciplinary teams, including health professionals, occupational therapists, and care providers, to coordinate integrated care and support.

Requirements:

Qualified Social Worker with relevant professional registration (HCPC).
Previous experience working in Adult Services, preferably within a Short Term Assessment and Reablement Team or similar setting.
Strong assessment and intervention skills, with the ability to develop and implement effective support plans.
Excellent communication and interpersonal skills, with the ability to build rapport and establish effective working relationships with service users, families, and colleagues.
Willingness to conduct community visits as part of the role.
