Senior Practitioner | Adults Assessment / Transition Team |London Borough of Hillingdon

• Qualified Social Worker required to join a statutory Assessment / Transition Team
• Office base: Hillingdon - Uxbridge
• To be considered, you must have a social work qualification and full SWE registration as well as over 3 years post qualified experience gained in a Hospital setting
• £36ph
