Job Description

Are you a dedicated and experienced Adults Qualified Social Worker looking for a rewarding opportunity in Sheffield? Key Responsibilities:

Conduct assessments of adults with care and support needs, identifying risks and strengths to develop personalized care plans.
Provide ongoing support and intervention to adults and their families, ensuring their voices are heard and their choices respected.
Work collaboratively with multidisciplinary teams, including health professionals, housing providers, and community organizations, to coordinate holistic care and support.
Maintain accurate and up-to-date records using Liquid Logic or similar case management systems, ensuring compliance with data protection and confidentiality policies.
Advocate for individuals rights and entitlements, ensuring they have access to appropriate services and resources to meet their needs.

Requirements:

Qualified Social Worker with relevant professional registration (HCPC).
Previous experience working as a Social Worker in an Adults Services setting, with a sound understanding of relevant legislation and policies.
Proficiency in using Liquid Logic or similar case management systems is highly desirable.
Excellent assessment and intervention skills, with the ability to develop and implement person-centered care plans.
Strong communication and interpersonal skills, with the ability to build rapport and establish effective working relationships with service users, families, and colleagues.
