Job Title: Social Worker - Hospital Discharge

Location: Somerset, United Kingdom

Hourly Rate: £38

Are you a qualified Social Worker with experience in hospital discharge planning? Were seeking a dedicated individual to join our team in Somerset, specifically at YDH, to support and facilitate smooth discharge processes for patients.

Key Responsibilities:

Conducting assessments of patients needs and circumstances to determine appropriate discharge plans.
Collaborating with multidisciplinary teams, including medical staff, nurses, and community support services, to coordinate safe and timely discharges.
Providing support and guidance to patients and their families throughout the discharge process, addressing concerns and facilitating access to necessary resources.
Liaising with external agencies and community organizations to arrange post-discharge support and services, ensuring continuity of care.
Documenting assessments, discharge plans, and interventions in accordance with professional standards and organizational protocols.
Requirements:

Qualified Social Worker with relevant registration (e.g., HCPC).
Proven experience in hospital discharge planning within a social work context.
Excellent communication and interpersonal skills, with the ability to engage effectively with patients, families, and multidisciplinary teams.
Strong organizational skills and the ability to prioritize workload effectively in a fast-paced environment.
Knowledge of relevant legislation, policies, and procedures governing social work practice in healthcare settings.
Flexibility to adapt to changing circumstances and work collaboratively within a team.
Additional Information:

This is a temporary position with an hourly rate of £38.
The role is based at YDH in Somerset, United Kingdom.
Hours of work may vary based on service needs, including evenings and weekends.
Travel within the Somerset area may be required as part of the role.
