Job Title: Adults Social Worker - County Team
Location: Hereford, Herefordshire
Salary: £35 per hour ltd
Hours: Full-time, 37 hours per week

Herefordshire Council is seeking a dedicated and experienced Social Worker to join our County Team, based in the heart of Hereford. This is a fantastic opportunity to work in a small, dynamic team where you will have the chance to make a real difference in the lives of vulnerable adults.

About Us:
The County Team operates from the Herefordshire Council offices at Plough Lane, with a hybrid working model that allows for two days in-office and three days remote work. This setup fosters collaboration, support, and team cohesion while providing flexibility and work-life balance.

Key Responsibilities:

Assessing the needs of service users in accordance with the legislative framework, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
Supporting individuals with a range of needs, including Learning Disabilities, Mental Health, Physical Disabilities, and housing issues such as hoarding and homelessness.
Facilitating hospital discharges and coordinating multi-agency support for service users.
Collaborating with the Council's Legal Team on Court applications for Deprivation of Liberty Safeguards (DoLS) Appeals, Community DoLS, and Deputyship Applications.
Working closely with local ICB, CHC team, Police, Health colleagues, and other multi-agency teams to ensure comprehensive support for service users.
Undertaking out-of-county work in specialist hospital and community settings as required.
Requirements:

Qualified Social Worker with relevant experience in adult social care.
Knowledge of relevant legislation and frameworks, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
Strong assessment and communication skills, with the ability to work collaboratively with multi-disciplinary teams and service users.
Ability to manage a varied caseload and work autonomously.
Willingness to travel and undertake out-of-county work when necessary.
