Are you an experienced Qualified Social Worker seeking a rewarding opportunity to support individuals transitioning between different stages of care? Join our Transition Team in Leicester and play a vital role in facilitating smooth transitions and improving outcomes for vulnerable individuals. We have both full-time and part-time positions available to suit your schedule.

Key Responsibilities:

Conduct thorough assessments of individuals needs and circumstances, with a focus on those transitioning between different care settings, including from hospital to community care.
Develop and implement robust transition plans in collaboration with service users, their families, and other professionals, ensuring continuity of care and support.
Provide advocacy and support to individuals involved in Mental Health Act (MHA) assessments and court proceedings, ensuring their rights and preferences are respected.
Facilitate timely and safe discharges under Section 117 of the Mental Health Act, coordinating with relevant agencies and stakeholders to ensure appropriate aftercare arrangements are in place.
Work closely with multidisciplinary teams, including health professionals, housing providers, and voluntary organizations, to coordinate seamless transitions and promote integrated care delivery.

Requirements:

Qualified Social Worker with relevant professional registration (HCPC).
Proven experience working within a Transition Team or similar setting, with a focus on supporting individuals with complex needs during periods of transition.
Sound knowledge and understanding of Mental Health Act (MHA) processes, including experience with Mental Health Capacity Assessments (MCHAs) and court proceedings.
Familiarity with Section 117 aftercare provisions and experience coordinating discharges and follow-up support.
Excellent communication and interpersonal skills, with the ability to build rapport and establish effective working relationships with service users, families, and colleagues.
