Are you a skilled and compassionate Qualified Social Worker with experience in handling complex cases? Key Responsibilities:

Undertake comprehensive assessments of the needs of adults requiring social care support, including those with complex needs such as Vulnerable Adult Risk Management (VARM), Safeguarding (SG), and Court of Protection (CoP) cases.
Develop and implement care plans in collaboration with service users, their families, and other professionals, ensuring the provision of person-centered and holistic support.
Provide ongoing support and intervention to adults and their families, advocating for their rights and ensuring access to appropriate services and resources.
Work collaboratively with multidisciplinary teams, including health professionals, housing officers, and community organizations, to coordinate integrated care and support plans.
Maintain accurate and up-to-date records of assessments, interventions, and outcomes in compliance with professional standards and organizational policies.

Requirements:

Qualified Social Worker with relevant professional registration (HCPC).
Substantial experience working as a Social Worker with adults in a statutory or voluntary sector setting.
Proficiency in handling complex cases, including those involving VARM, SG, and CoP, with a demonstrated ability to assess risks and develop effective intervention strategies.
Strong communication and interpersonal skills, with the ability to build rapport and establish positive relationships with service users, families, and colleagues.
Knowledge of relevant legislation, policies, and procedures governing adult social care practice.
