Key Responsibilities:

Conduct comprehensive assessments of the needs of older people, taking into account their physical, emotional, and social well-being.
Develop and implement care plans in collaboration with service users, their families, and other professionals, ensuring the promotion of independence and dignity.
Provide ongoing support and guidance to older people and their families, advocating for their rights and ensuring access to appropriate services and resources.
Work closely with multidisciplinary teams, including healthcare professionals, community organizations, and local authorities, to coordinate holistic care and support for older people.
Maintain accurate and up-to-date records of assessments, interventions, and outcomes in accordance with professional standards and organizational policies.

Requirements:

Qualified Social Worker with relevant professional registration (HCPC).
Previous experience working with older people in a social care setting, ideally within a statutory or voluntary sector organization.
Strong assessment and case management skills, with the ability to develop and implement person-centered care plans.
Excellent communication and interpersonal skills, with the ability to build rapport and establish effective working relationships with service users, their families, and colleagues.
Sound knowledge of relevant legislation, policies, and procedures related to social work practice with older people.
