Job Description

We are currently seeking two full-time qualified social workers to join our Adult Central Team. This hybrid role involves spending two days per week in our Civic office, where you will undertake duty functions, and the remaining time conducting Care Act assessments and reviews in the community. This unique position offers a varied and fulfilling experience, allowing you to make a real difference in the lives of vulnerable adults.

Key Responsibilities:

Conducting comprehensive Care Act assessments and reviews for adults in need of support.
Working collaboratively with multidisciplinary teams to develop and implement care plans that promote independence and well-being.
Providing expert advice and guidance to service users, their families, and carers.
Undertaking safeguarding investigations and implementing appropriate safeguarding measures.
Maintaining accurate and up-to-date records in accordance with professional standards and legal requirements.

Requirements:

Qualified Social Worker with relevant registration (HCPC).
Proven experience working within adult social care, including undertaking Care Act assessments and reviews.
Strong knowledge of relevant legislation, policies, and procedures within social work.
Excellent communication and interpersonal skills, with the ability to build rapport and establish trust with service users.
Ability to work autonomously and effectively manage a varied caseload.
Commitment to continuous professional development and adherence to professional standards of practice.
