Hours: Full-time (08:00 - 16:00)

Midlands Partnership NHS Foundation Trusts Unplanned Care South East division is seeking a dedicated Band 5 Social Worker to join their dynamic team at Samuel Johnson Community Hospital in Lichfield. This is an exciting opportunity to make a meaningful impact in a collaborative and supportive environment.

Role Overview:
As a Band 5 Social Worker, you will play a vital role in supporting the Unplanned Care South East team in delivering high-quality care to adults in the Midlands community. You will be responsible for conducting assessments, developing care plans, and providing intervention and support to individuals and their families. This role offers a unique opportunity to work in a hybrid setting, combining both remote and onsite work to ensure optimal flexibility and efficiency.

Key Responsibilities:

Conducting comprehensive assessments of service users needs and risks.
Developing and implementing tailored care plans in collaboration with multi-disciplinary teams.
Providing direct support and intervention to service users and their families, promoting independence and wellbeing.
Maintaining accurate and up-to-date records in accordance with trust policies and procedures.
Participating in team meetings, case conferences, and other collaborative activities to ensure coordinated care delivery.
Requirements:

Qualified Social Worker registered with Social Work England.
Previous experience working with adults in a social care setting, preferably within unplanned care or hospital discharge planning.
Strong assessment and intervention skills, with the ability to manage complex cases effectively.
Excellent communication and interpersonal skills, with the ability to engage effectively with service users, families, and colleagues.
Commitment to delivering high-quality, person-centered care within a multi-disciplinary team environment.
Flexibility to work in a hybrid model, including both remote and onsite work as required.
