Job Description:
We are currently seeking a Qualified Social Worker (QSW) to join our Adult Services team. The successful candidate will be responsible for assessing the needs of adults requiring social care support, developing care plans, and coordinating services to ensure the well-being and safety of our service users.
Key Responsibilities:
Conducting comprehensive assessments of service users needs, including risk assessments and safeguarding concerns.
Developing and implementing person-centered care plans in collaboration with service users, their families, and relevant stakeholders.
Providing advice, support, and advocacy to service users to empower them in making informed decisions about their care.
Collaborating with multidisciplinary teams and external agencies to coordinate and deliver appropriate support services.
Maintaining accurate and up-to-date records in accordance with statutory requirements and organizational policies.
Participating in case conferences, reviews, and other meetings as required.
Requirements:
Degree in Social Work or equivalent qualification.
Registration with Social Work England.
Proven experience in working with adults in a social care setting, preferably within adult services.
Sound knowledge of relevant legislation, policies, and procedures related to adult social care.
Strong assessment, communication, and interpersonal skills.
Ability to work effectively both independently and as part of a team.
Commitment to continuous professional development and reflective practice.
