Nottinghamshire - Adult Occupational Therapist (HYBRID)

Job Description

We are currently seeking a highly motivated and qualified Occupational Therapist to join our team in Nottinghamshire. The successful candidate will be working within the short-term assessment flats located in Newark. This role involves working with a diverse range of individuals who have been transferred to the flats for reablement purposes, aimed at supporting them to return home and avoiding the need for long-term care.

Key Responsibilities:

Conduct assessments to determine individuals needs and develop personalized reablement plans.
Provide therapeutic interventions aimed at improving individuals independence and quality of life.
Work closely with an onsite care team to deliver holistic care and support to individuals residing in the flats.
Collaborate with other healthcare professionals and external agencies to ensure coordinated and effective care delivery.
Monitor progress and adjust treatment plans as necessary to achieve optimal outcomes for individuals.

Requirements:

Bachelors or Masters degree in Occupational Therapy.
Registration with the Health and Care Professions Council (HCPC).
Previous experience working as an Occupational Therapist, preferably in a community or reablement setting.
Strong assessment and intervention skills.
Excellent communication and interpersonal skills.
Ability to work effectively within a multidisciplinary team.
