Job Description

We are currently seeking an experienced and compassionate Social Worker to join our Ageing Well CAT in Bassetlaw. The successful candidate will play a pivotal role in delivering high-quality social work support to older adults and their families within the community.

Key Responsibilities:

Conducting comprehensive assessments of service users needs, including risk assessments and care planning.
Providing person-centered support and advocacy to older adults to enable them to live independently and access appropriate services.
Collaborating with multidisciplinary teams, including healthcare professionals and community organizations, to coordinate and deliver integrated care plans.
Offering emotional support and practical advice to service users and their families, promoting their well-being and quality of life.
Ensuring compliance with relevant legislation, policies, and procedures, including safeguarding and mental capacity assessments.

Requirements:

Qualified Social Worker registered with the Social Work England (SWE).
Proven experience in adult social work, preferably with older adults and/or within a community setting.
Strong assessment and care planning skills, with the ability to develop tailored interventions based on individual needs.
Excellent communication and interpersonal skills, with the ability to build rapport and establish trust with service users and their families.
A car driver with access to a vehicle, as the role may involve travel within the Bassetlaw area.
