Job Description

We are seeking a motivated and experienced Adults Health and Commissioning Qualified Social Worker (QSW) to join our dynamic team at Addenbrookes Hospital in Cambridgeshire. The successful candidate will play a crucial role in providing high-quality social work support to adults accessing our services.

Key Responsibilities:

Conducting assessments and developing care plans for adults in need of support.
Providing advocacy and support to service users to ensure their voices are heard.
Collaborating with multi-disciplinary teams to coordinate care and support services.
Liaising with external agencies and stakeholders to facilitate holistic care.
Contributing to the ongoing development and improvement of services through feedback and evaluation.

Requirements:

Qualified Social Worker with relevant registration and professional accreditation.
Proven experience in adults health and commissioning social work, preferably within a hospital or healthcare setting.
Strong assessment and case management skills.
Excellent communication and interpersonal skills.
Ability to work autonomously and as part of a team.
Flexibility to work a minimum of 3 days in the office, with potential for 5 days based on service demand.
Commitment to upholding professional standards and ethics.
