Job Description
LD Team: Request for 2 x agency social worker posts to be funded out of the existing staffing budget. Purpose of recruitment is to assess the 50 young people aged 16+ currently waiting to be allocated in the current adult locality teams. These workers will help to assess these young people, commission any support required post 18, and work alongside the young person, their family and the SEND team in Sefton to look at post-school education, and/or employment and work opportunities. Request for 2 x agency social workers for 6 month contracts. Experience in transitions (preparing for adulthood). Essential criteria: learning disability background.
